A Lump or Swelling in the Neck: The most common symptom of thyroid cancer is a noticeable lump or swelling in the neck. This lump may be painless and can be felt when touching the neck.

What is Thyroid Disease?

The two most common forms of thyroid disease are:

Who Is At Risk for Thyroid Cancer?

Thyroid Disease: Signs, Symptoms and Treatment

Thyroid disease occurs when the thyroid, a butterfly-shaped gland located in the neck just about the collar bone, stops working properly. The thyroid works to release hormones that help to control the functions of your body, including how fast your heart beats, metabolism, bone health, energy levels, and other functions. When thyroid disease occurs, you may have too much thyroid hormone, called hyperthyroidism, or too little thyroid hormone, a condition called hypothyroidism.

Symptoms of Thyroid Disease

Thyroid disease can occur in various ways. Here is a look at some of the conditions and symptoms you may have:

Hyperthyroidism symptoms include:
• Sweating
• Irregular heartbeat
• Weight loss
• Protruding eyes
• Anxiousness

Hypothyroidism symptoms may include:
• Weight gain
• Fatigue
• Depression
• Stunted growth
• Abnormal bone development

Hashimoto’s thyroiditis symptoms may include the following:
• Goiter, or swelling in the neck due to an enlarged, inflamed thyroid gland

In addition to these conditions, some people develop thyroid tumors and thyroid cancer. These often begin with the presence of a growth in the area of the thyroid.

Your doctor will determine what’s occurring through a series of blood tests and a physical examination.

What Thyroid Treatment Is Available?

Numerous thyroid treatment options exist, and each depends on the condition and the severity of the disease. Often, thyroid hormone therapy is the first step, which allows doctors to adjust how much hormone is in the body.

For those who develop tumors, it may be necessary to have surgery to remove them. In addition, some people may have thyroid disease that requires the removal of the gland. If this occurs, you will remain on daily medication to replace the lost hormones for the rest of your life.

A second reason for surgery is if the thyroid becomes enlarged, it forms a large lump at the front of the neck, commonly called a goiter. Besides being unsightly, a goiter can push on the esophagus, making it difficult to swallow or breathe.

The third main reason for surgery is when thyroid glands develop small lumps called nodules. In rare cases these are cancerous, and in most cases the entire gland is removed. Most of these nodules have no symptoms, but are found during routine imaging tests like MRI scans for back or neck problems.

Thyroid disease treatment can be very effective, especially when provided by a specialist in the field. Endocrinologists typically provide this type of help and have a wide range of thyroid treatment options to manage thyroid conditions effectively.

Treatment Options for Thyroid Cancer

The thyroid is an organ that produces hormones that play a role in the various functions of your body. Your thyroid can help control how fast your heart beats, how your metabolism functions, and how digestion works. This organ, located in the front portion of the neck near the base, is quite small but must work properly to keep you healthy. Some people develop thyroid disease, and it’s not uncommon, with more than 20 million people in the U.S. with this disorder. Some people develop thyroid cancer.

Signs and Symptoms of Thyroid Cancer

Not everyone who has thyroid disease has thyroid cancer. Surgical Oncologist, Dr. Ana K. Wilson with Premier Surgical at Fort Sanders Regional in Knoxville explains, “Thyroid cancers can often be asymptomatic and are identified on ultrasound. They can also present as a lump in the neck either in the middle of the neck where the thyroid gland is, or sometimes on the side of the neck, where the lymph nodes are.”

Thyroid cancer can develop when there are changes in the DNA within the cells of the thyroid. These mutations grow and multiply, accumulating in a mass called a tumor. That tumor can move into other areas and spread through the lymph nodes to other areas of the body, such as the lungs, bones, and other areas.

How to Treat Thyroid Cancer

There are numerous types of cancer of the thyroid, and each one requires a different treatment method. After ultrasounds and getting a better idea of the location of the cancer, doctors can create a plan for treating it. This may include a thyroidectomy, surgery to remove all or most of the thyroid as a way to remove the cancer cells. There are medications available that can mimic the function of the thyroid if it is no longer present.

If surgery is not an option, radioactive iodine may be used as a way to kill the thyroid cells. This may also be done after surgery.

Some people may also benefit from the use of targeted drug therapy that blocks specific chemicals to cause cancer cells to die. Radiation therapy, chemotherapy, and other treatments may also be available to kill any remaining cancer cells that develop.

Fellowship-trained Surgical Oncologist Treats Thyroid Cancer

After a career in the United States Air Force, Jesus Navarro has developed a can-do and will-do attitude. If something needs to be taken care of, he gets the job done.

So it’s no surprise he was stoic when he received a diagnosis of thyroid cancer. “I really had no big reaction. There was something in me that needed to be taken care of,” Navarro says.

In this case, he got that “something” taken care of at Fort Sanders Regional Medical Center.

A Growing Concern

As Navarro has moved away from military life and into retirement, he’s been pleased to swap his uniform for gardening clothes and discovered a gift for making things grow. Along with the tomatoes, potatoes and onions that have graced his garden through recent years, he’s become a fan of flowers.

“I like to buy flowers that are distressed and bring them back to life,” Navarro says. “I’ve had pretty good luck with that, and I like to grow flowers from bulbs, also.”

In December 2021, this peaceful life was interrupted by a lump growing on his neck. A Veterans Administration doctor recommended an ultrasound and CT scan, and Navarro soon received the news that the lump was a malignant tumor.

Choosing Fort Sanders Regional

Like the distressed plants he carefully nurtured, Navarro needed detailed and compassionate care to help him preserve his quality of life. That care came from Ana Wilson, DO, a surgical oncologist with Premier Surgical Associates at Fort Sanders Regional who is fellowship-trained in endocrine surgery.

“If the cancer is fairly small and is what we call a well-differentiated cancer, we can remove either half of the thyroid or the whole thyroid depending on the size of the lesion,”  Dr. Wilson says. “If there are any abnormal- appearing lymph nodes in the lateral parts of the neck, sometimes we have to perform a neck dissection to remove all of the lymph nodes on that side.”

Thyroid procedures can be delicate to perform. Dr. Wilson says the thyroid gland lies directly on top of the trachea, and there is also a risk of injury to the recurrent laryngeal nerve.

“That’s the nerve that controls the vocal cords,” Dr. Wilson says. “It has to be carefully identified during the surgery, and protected to avoid permanent hoarseness in the patient.”

Other risks include bleeding, because the thyroid is a very vascular organ requiring continuous blood circulation, and decreased calcium levels.

“The parathyroid glands lie directly next to the thyroid, and they control calcium levels in our bodies,” Dr. Wilson says. “If those are damaged or accidentally removed during the surgery, it can cause low calcium levels in the postoperative period.”

Regaining Command of His Life

Navarro’s papillary (well-differentiated) cancer was removed with a complete thyroidectomy, and a neck dissection to catch lymph nodes where cancer had spread. With Dr. Wilson’s advanced training in endocrine surgery and special skill in removing cancerous tumors on the thyroid, Navarro’s procedure was a success.

“The surgery went excellent, and I highly praise Dr. Wilson,” Navarro says. He also praises her for her approach to caring for him before and after surgery. “It was just very patient-oriented, and she has a lot of compassion and understanding.”

Navarro has been prescribed medication and vitamins to take over the function of the gland that was removed and is doing well now. Dr. Wilson says survival rates for this most common type of thyroid cancer are usually very good.

“As with all cancers, prevention and early detection are important,” Dr. Wilson says. “If a person notices an unusual lump in the neck that wasn’t there previously or starts to have trouble swallowing, or has any other concerning symptoms of compression in the neck, they should alert their doctor and think about getting an ultrasound.”

Your thyroid is a gland located in your neck, around your windpipe. Although it’s small, it secretes hormones that control many bodily functions, including your digestion, metabolism, and brain development.

When your thyroid makes either too little or too much of these hormones, you may develop conditions called hypothyroidism or hyperthyroidism, which cause different symptoms.

There are various causes for these endocrine disorders, and the surgeons of Premier Surgical Associates of Knoxville, and Cleveland, TN, remind you that treatment will depend on what’s causing the problem and the severity of your symptoms.

Common thyroid issues
The most common kinds of thyroid disease are hypothyroidism and hyperthyroidism.

Hypothyroidism
When your thyroid gland isn’t producing enough of the hormones your body needs, this is called hypothyroidism, often referred to as an underactive thyroid. It can cause exhaustion, weight gain, and constantly feeling cold.

Hypothyroidism could occur when:

• Your body is exposed to radiation through cancer treatments
• You’ve had thyroid cancer surgery previously
• You have Hashimoto’s disease, an inherited autoimmune disease

Hyperthyroidism
The inverse of hypothyroidism, if your thyroid produces too much hormone, you have hyperthyroidism – an overactive thyroid. This can leave you feeling anxious and exhausted, with tremors and insomnia.

The causes of an overactive thyroid include:

• Graves’ disease – an inherited autoimmune disease
• Excess levels of iodine in your body
• Thyroid cancer
• Nodules – fluid-filled growths on your thyroid gland

What treatments are available?
For an underactive thyroid gland, the best treatment is usually to replace the deficient hormone with a synthetic version called Levothyroxine, to allow levels to come back up to a normal range.

With an overactive thyroid, there are different treatments available. Usually, anti-thyroid medications can help reduce the production of the hormone, with beta-blockers available to help combat symptoms such as a racing heart.

In more severe cases, radioactive iodine may prevent the thyroid from producing as much of the hormone. With an enlarged thyroid, surgery to remove it could be an option.

Your thyroid is the butterfly-shaped gland that sits at the base of your neck. Wrapping the trachea, the thyroid gland is responsible for producing hormones that regulate your blood pressure, heart rate, body temperature, and weight.

When cells in your thyroid gland undergo changes (i.e. mutation), they can grow and multiply rapidly. This is how thyroid cancer develops.

When left unchecked and/or untreated, these abnormal cells can invade nearby tissue and spread to other parts of the body.

The signs and symptoms to watch out for
During the early stages, thyroid cancer may not show any sign or symptom. In fact, in many cases, thyroid cancer is only detected during a routine neck examination or during an image scan performed for other conditions.

For people who are symptomatic (showing signs and symptoms) and as the disease progresses, an unusual lump or swelling in the neck may be felt. Although most nodules in the neck are benign, it’s still best to see a doctor right away to rule out thyroid cancer.

Other early signs and symptoms to watch out for include hoarseness in voice, fatigue, swollen glands in the neck, and a persistent cough that’s not caused by colds.

In some cases, there can also be neck pain, difficulty in swallowing, and breathing problems.

If you experience ANY of these, schedule a doctor’s appointment right away. It’s better to err on the side of caution.

Before a diagnosis can be made, your doctor will perform a thorough physical check and will run some tests. This includes a tumor marker test that will check high levels of certain hormones.

Treating thyroid cancer
Treatment for thyroid cancer can vary, depending on the type and stage of thyroid cancer.

When 48-year old Melissa Bingham’s physician recommended surgery to remove her thyroid and an enlarged, but noncancerous nodule on her thyroid, the Clinton, Tennessee woman cringed at the thought of a large scar across her neck.

“As a woman I thought, ‘Do I really want to look like I’ve had my throat cut?’” confesses Bingham. Conventional open thyroid surgery usually involves a long neck incision for most patients and a visible scar.

Bingham shared her concerns with Dr. David Harrell, the Premier Surgical surgeon her physician referred her to for the procedure.  Bingham was delighted when Dr. Harrell, who has performed more than 700 surgeries using a robotically-assisted system, suggested that she could be a candidate for a robotic thyroidectomy.

“We discussed it and I watched a robotic thyroid surgery online that was performed in Korea,” explains Bingham.  “Dr. Harrell had already been trained to do this surgery robotically and I felt confident in his ability.”

Dr. Harrell says experience in thyroid procedures and his frequent use of robotic surgery techniques were a natural combination for performing a robotic thyroidectomy.  “It involves the same robotic instruments we already use every day and offers people a way to have a thyroid safely removed without a visible neck scar.”

With robotically-assisted surgery, robotic instruments and a 3-D camera are inserted into the surgery site using tiny incisions. The surgeon controls the instruments from a computer console. The camera and instruments give the surgeon a detailed view and excellent precision. Patients have less scaring, pain and a quicker recovery versus traditional surgery.

On March 5, 2020, Dr. Harrell successfully performed Bingham’s thyroidectomy with the da Vinci Surgical System at Tennova Healthcare’s North Knoxville Medical Center. He used small incisions in her arm pits and chest to remove a golf-ball sized mass that included Bingham’s thyroid and the nodule. It was the first robotic thyroidectomy performed in the state of Tennessee.

Bingham was surprised at the reaction she received after the procedure. “I had no stiches or drains. When people saw me they couldn’t believe I had just had thyroid surgery!”

Dr. Harrell is pleased to have performed the first robotic thyroidectomy in the state and expects the use of this innovative technique to increase.

“Thyroid disease is becoming more common. We’re doing more screenings now and finding thyroid nodules more frequently,” explains Dr. Harrell. “I’m excited to offer this new surgical option to patients.”

Melissa Bingham agrees. “I would highly recommend it to anyone. Given the choice, I’d definitely have a little temporary tenderness across my upper chest than a big scar across my neck.”

General Surgeon Dr. David Harrell of Premier Surgical Associates successfully performed Tennessee’s first “hidden scar” robotic Thyroidectomy on March 5, 2020, at Tennova Healthcare’s North Knoxville Medical Center.

Unlike traditional open thyroid surgery that involves a long neck incision and a visible scar, the patient’s thyroid and a large nodule were removed through her armpit and chest via a few tiny cuts.

The procedure was performed using the da Vinci Surgical System. It features a 3-D camera and robotic instruments that the surgeon controls from a computer console. The system gives the surgeon a detailed inside view of the surgery site and excellent precision. Patients generally have less scaring, pain and a quicker recovery versus traditional surgery.

Robotically-assisted surgery has been used for years in the United States for prostate, heart and gynecological procedures, but robotic thyroidectomy has only been performed at a small number of medical centers in the U.S.

The thyroid is a gland in the neck that helps control the body’s metabolism. Thyroid nodules are common and occur in some 30 percent of Americans. Dr. Harrell is pleased to have performed the first robotic thyroidectomy in Tennessee state and expects the use of this innovative technique to increase.
